Humana Dental PPO High

The Humana Dental PPO High Plan allows members to visit any licensed dentist, while offering the greatest cost savings when using an in-network dentist. In-network dentists have contracted fees, which help reduce out-of-pocket costs. Members may also use out-of-network dentists, but members may pay more.

The PPO High Plan is designed for members that may need more extensive dental care. Covered services are paid at different benefit levels depending on the type of service and whether care is received in-network or out-of-network. Preventive services are covered at 100% in-network and 90% out-of-network with no deductible. Basic services are covered at 80% in-network and 70% out-of-network after the deductible, and major services are covered at 50% in-network and 40% out-of-network after the deductible.

The plan includes a $50 individual deductible and $150 family deductible, which applies to basic and major services. The plan also includes a $2,000 annual maximum per member, excluding orthodontia. Adult and child orthodontia services are covered at 50%, with a $2,000 lifetime orthodontia maximum.

When members use an out-of-network dentist, benefits are based on the plan’s usual and customary (U&C) charges at the 90th percentile, rather than contracted in-network fees. Because out-of-network dentists are not part of the Humana network, they may charge more than the amount recognized by the plan, and members are responsible for any difference in addition to applicable deductible and coinsurance. Members should review the benefit schedule for covered services, frequency limits, exclusions, and applicable benefit percentages.

Summary of Benefits
